What is this effect called?

Recently I started performing an effect where you get 2 spectators to sign 2 different selections then you fuse those 2 cards into one. I do it using a method I come up with. It is similar to how Adam Wilber performs it in this video around the 4 minute mark.
What is the name of the original card-fusing effect and in what book/DVD can I find the original handling of this trick?

Generally, the effect is known as Doc Eason's Anniversary Waltz. It is available in a booklet (published around 1994), on Doc Eason's Bar Magic DVD Volume 3 and on an L&L World's Greatest Magic DVD titled "Anniversary Waltz" (with several variations). Doc is credited with the presentation. Doc's handling was based on Chris Carter's "Cold Fusion" effect that was written up in the September 1990 issue of Linking Ring. That routine was a variation of Wesley James's "Forgery" published in his Stop Fooling Us! lecture notes (1989).
