You can find it (Or at least the original {I think} version of it) in Chris Kenner's Totally Out of Control.

Ah, Sean beat me to it. Yes, the basic method behind the effect can be found in "Totally Out of Control," under the title "Paint by Numbers," but the handling is not exactly the same, nor is the presentation. The variation that I performed is nearly the same handling as Doug Conn's take on this effect, which astonished me the first time I saw it on youtube.
