Does anyone still do fast and loose?

Yeah, kinda. I mess around with both the chain and belt versions. I haven't seen anyone perform it live but I have thought about adding it to my busking repertoire. As a magic expose I can't make it too compelling.

Is that the carnival game where you have to pick which hole is on the inside, and you can never win? I honestly have never performed it, nor seen anyone perform it, save one video on YouTube.

I saw Ricky Jay live in 2014 try to do it at his "Congress of Wonders" but he forgot how to do it. That was interesting.
Whit Haydn and Chef Anton (School for Scoundrels) have a 'Fast & Loose' CD/book set, which I have and recommend.
I think it plays well with Three-Card Monte and the Shell Game.
