Well risk is a huge entertainment factor, which is why they do it. Why on earth would someone stick a nail in their nasal passage, because of the gross impossibility that something bad could happen.
Also, there is one version out that is practically foolproof. You would have to be on LSD and drunk to not get it right. I don't know the name but it is practical and incredibly safe to use it.
I still have to shake my head at the "hide the spike in the cup" trick. There are some awesome videos of that going wrong on Youtube. Its great when it works...but why try something when there is that 1% chance that something could go wrong?
